In order for the baby’s head to pass through the vaginal cavity and be easily delivered, a part of the skin between the vaginal inlet and the anus (perineum) is cut with surgical scissors and the vaginal inlet is enlarged to make delivery easier, called perineal incision.
It is not necessary to perform perineal incisions normally during natural delivery. In the past 20 years in the United States, during natural delivery through natural birth canal, some hospitals usually performed perineal incision, but nowadays perineal incision has no real benefit for delivery, a study found.
So, in 2000, only about 33% of them received perineal incision and delivered. Natural delivery without perineal incision does not tear the skin of the perineum of many delivery parts.
Because it can be sutured, a perineal incision is usually not recommended these days. Because the fetus is in distress (fetal distress), it is necessary to proceed with the stage 2 delivery stage quickly, or when the delivery woman has health problems such as heart disease and needs to proceed quickly, or when delivering a larger baby than usual,
Or, if the vaginal birth canal is widened by perineal incision when delivering a premature baby, the baby’s head can come out more easily and only the second stage can be finished quickly.
In order to prevent the perineum from being cut, the vaginal birth canal can be widened by hand when delivering, and sometimes even if the perineal part is cut, only a little bit may occur. After delivery, the part of the perineum that was spontaneously slit or the part of the perineum that was cut by perineal incision can be repaired by suturing with a surgical needle to recover the surgical site easily.
The slit perineum can be sutured with a suture that can be absorbed when performing suture treatment, or it can be sutured with a suture that is not absorbed. If sutures are sutured with non-absorbable sutures, the stitches must be removed a few days after suturing.
Perineal incision surgery is a relatively simple operation and heals relatively well. Ask your doctor for more information about perineal incision surgery. Copyright ⓒ 2014 John Sangwon Lee, MD, FAAP
