임신 28주부터 임신 40주까지의 임신 기간을 편의상 산과에서는 임신 제 3 삼개월기라고 한다. 임신 제 2 삼개월기간 동안 태아는 무럭무럭 성장 발육해서 임신 제 3 삼개월기의 마지막 주 즉 임신 40주에 출생한 만삭 신생아의 출생시 체중은 3-3.6kg이 되는 것이 보통이다. 임신 제 3 삼개월 기간동안 임신부의 체중은 임신으로 4.5-5.5kg까지 늘 수 있다 (미국의 통계에 의하면).
임신 40주는 임신 제 3 삼개월기의 열셋째 주이다. 임신 38주 이후에서 임신 41주 전에 출생된 아기는 만삭아라고 부른다.
임신 35~36주부터 매주마다 분만 전 임신검진을 받는 것을 권장한다. 검진 받을 때마다 아기가 얼마나 큰지 자궁내 아기가 어떤 형태로 위치하고 있는지 체크해본다. 임신검진을 받을 때 통상적인 피검사를 하고 혈압을 측정해보고 신생아 패혈증이나 뇌수막염을 일으킬 수 있는 B형 연구균감염을 예방하는 검사도 받을 수 있다.
자궁이 매 30초마다 탄탄해지게 수축되고 이완되는 자궁수축이 생길 수 있다. 이렇게 자궁이 수축될 때 통증은 없다. 출산을 하기 위한 자궁수축이라기보다 출산을 준비하는 예비 자궁수축이라고 할 수 있다. 이런 자궁수축을 브락스톤 히크스 증후라고 한다. 브락스톤 히크스 증후군으로 자궁에 수축이 생길 때는 자궁의 경관이 벌어지지 않는다.
임신 40주에 이르면 아기가 든 자궁이 골반 강을 차지하고 복강의 대부분을 차지하기 때문에 인신부는 상당히 불편함을 느낀다. 그리고 수면부족, 여기저기에 생기는 통증, 피로, 분만에 대한 두려움 등으로 임신 우울증이 생길 수도 있다. 그와 반대로 어떤 임신부는 희망에 가득 차 있을 수 있다.
그 동안 임신으로 희망도 갖고 기쁨도 많았다. 또 고생도 많이 했다. 이제 출산할 계획을 완수하는 때가 왔다. 출산 예정일에 출산하는 경우는 전 임신 중 5%밖에 되지 않는다. 사실은 출산 예정일의 2주 전후에 대부분 출산하는 것이 보통이다. 하여튼 그동안 건강한 아기를 임신하고 건강하게 출산하는 임신부 여러분들께 축하를 드린다.
장차 있을 출산에 관한 계획을 하고 어떤 방법으로 출산을 할 것인가 어떤 병원에서 출산할 것인가 출산할 때 어떤 방법으로 진통을 덜을 것인가 등등에 관해서 연구하고 의사와 간호사 또는 출산을 도와줄 수 있는 사람과 상담한다.
임신 37주에서 40주 사이에 임신부들은 그야말로 기쁨과 두려움이 섞인 감정을 갖는 것이 보통이다. 임신 40주 이전 1~2주 동안에 임신부의 체중은 더 이상 늘지 않고 현상태로 유지되는 경우가 많다. 태아가 든 자궁이 골반강과 복부강의 대부분을 차지하기 때문에 숨쉬기도 상당히 곤란할 수 있다.
