Complicated labor: Emergency labor and delivery
Depending on the situation, contact a doctor, hospital emergency room, or 119 emergency department for assistance.
You can give birth in an emergency when you’re not expecting to give birth. I planned to deliver at the hospital instead of at home, but there are times when I am suddenly delivering at home.
There are even occasional cases of unexpected and sudden delivery on an airplane. Emergency labor can be called an emergency delivery when labor suddenly starts and gives birth.
If you don’t have time to go to the hospital that you have planned and set for a natural childbirth, you have no choice but to give birth at home or on the spot in a car.
In some countries, delivery is performed at home without the help of a medical professional, and in the United States, delivery is also performed at home with the help of a midwife. Also, you can sometimes read newspaper articles about emergency delivery in the car at home. Unexpectedly, emergency delivery can be done as follows: delivery, delivery department, newborn baby, and postpartum treatment.
Even if it seems that a baby will be born soon after an emergency delivery, in many cases, there are many times when you settle in a nearby hospital and have time to deliver at the hospital.
If you want to deliver unexpectedly and suddenly, do not panic, inform the doctor you have been taking care of by phone that you are about to deliver, and follow the doctor’s instructions for emergency delivery, or call the 119 paramedic for emergency help.
Or, call the hospital emergency room for help. At this time, if you have names, addresses, expected delivery dates, unusual health problems or complications from pregnancy, inform them and ask for help.
① Even if the labor begins suddenly and the baby is likely to be born soon, take the dispersal department to an appropriate nearby hospital using an emergency vehicle or other appropriate means of transportation, and do our best to receive the help of a doctor at that hospital and deliver more safely. Whether you have the time to do so or not, follow the directions of 119 medical paramedics or hospital emergency departments to help you deliver safely on site. If you can go to the hospital and deliver, take an ambulance to the hospital for delivery. If you don’t have time to go to the hospital and deliver, you can call a doctor, midwife, or nurse to come to the scene and ask them to help you deliver on the spot. And even if the baby is already born, call the doctor for help.
② Most pregnant women can deliver safely and naturally without the help of a doctor, nurse, or midwife. Even if the pain starts suddenly and you think you will be delivering soon, don’t panic and stabilize the pregnant woman first.
③ If the baby is likely to be born soon and there is no time for other help, the delivery department lays a towel or sheet on the floor, the floor, or the seat of a car in an appropriate place on the floor, with both legs bent and spread out. However, if the woman wants to deliver in a squatting position, the childbirth helps the childbirth to deliver in that position, and if they want to give birth while lying down, in the lying position. Teach them not to push the baby away with unnecessary force until the baby’s head is visible from outside the birth canal. If the baby’s head is visible from outside the birth canal, it must be pushed out.
At this point, you can breathe often just before pushing and then push the baby out with full force. When the baby’s head is in the vaginal opening of the birth canal and the uterus contracts, the delivery department teaches that it can push the baby down with force. If the baby’s head that came out of the birth canal is supported with both hands, it is common for the head to be delivered naturally.
When the baby’s head comes out of the birth canal, tell them not to push the baby anymore for a while, and quickly scrub the baby’s nose and the blood and mucus in his mouth with his fingers. If the umbilical cord is wrapped around the neck, it will also be loosened. It is common for babies to be delivered in the order of shoulders, trunks, hips and legs by holding the baby’s head slightly with both hands and pushing it slightly toward the floor. When the baby’s shoulders or hips come out of the birth canal and then stop, force the baby’s belly and push it out, and it will be delivered naturally. But there is no need to force it to be pulled out.
There may be amniotic fluid in the baby’s airways. Lay the baby’s head a little lower than the torso on the mother’s chest for a minute or two. Receive your baby on a clean towel, sheet, bed linen, or blanket. If the amniotic fluid erupts, the sheet or clothes may get wet and the surrounding area may become dirty.
Sheets that are soiled with inexpensive feces during delivery are replaced with clean sheets.
Don’t go to the bathroom by yourself.
④ People who help childbirth wash their hands thoroughly with soapy water as much as possible.
There is no need to wash the external genitalia of the delivery part.
⑤ Most of the delivery can be delivered naturally by the delivery department without help. However, if you seem to be having a hard time, you should take the delivery department to a nearby general hospital in a hurry.
⑥ If the baby born is covered with an amniotic membrane, tear the amniotic membrane carefully and walk.
⑧ Do not pull the umbilical cord with your hand so that the placenta in the womb comes out quickly. After the placenta comes out, wait for the umbilical cord to stop beating completely, hold the two parts of the umbilical cord with a sturdy thread, and then cut the part between the two parts of the umbilical cord that has been pulled with sterile scissors. Place the placenta in a bowl and handle it according to your doctor’s instructions.
⑨ If you don’t have a warm and clean blanket or sheet to wrap your baby in, wrap your baby in a coat, etc.
⑩ After delivery, gently rub the mother’s lower abdomen with hands to prevent bleeding from the uterus.
⑪ Cover the mother warmly with a blanket, etc.
⑫ Delivery is done at home only if it is impossible to deliver at the hospital. During or after delivery at home, you should seek help from a midwife or doctor. Contact your pediatrician and follow his instructions to take care of your baby’s health. Copyright ⓒ 2014 John Sangwon Lee, MD., FAAP
